Tuition for Day Classes

A registration fee of $250.00 is required with this application. However, you may pay the full amount when submitting this form if you wish.

I enclose $250.00 as an advance down payment toward the Floral Design Program. I understand that the balance is to be paid on the first day of class. If the above class is full my advance payment will be returned, or if requested, held for enrollment at a later date.

2012 Course Dates		Please Note: It is important that you "check" your preferred class date by using the check box beside your choice. This will assure your enrollment for that period.
